Home from Hospital service
After an operation or hospital treatment, returning home from hospital can be daunting for some elderly and vulnerable who may be concerned about how they will cope and where to get extra support. Our Home from Hospital care service makes the transition easier and can normally be arranged within days.
The service consists of a 24/7 personal alarm with alarm pendant, medical helplines and a regular wellbeing call to make sure you are settling back home OK. And the first 6 weeks are free!

To be eligible for the Home & Well service, you must have been discharged from hospital six weeks prior to point of enquiry. Home & Well service also includes returning home from respite care.
